Please be aware that Centrelink will cease your child’s enrolment if they have not attended OSHC for a period of 13 weeks or more. If this occurs, we will require a new CWA to be completed prior to your child recommencing at OSHC. The OSHC office will contact you directly regarding this.

Westside Christian College Bus Policy
- My child is booked to catch a bus only and in complying with the schools Bus policy I am required to sign this enrolment form. I/ We agree and understand that in the event that I am not able to meet my child at the designated bus stop or at my/our home my/our child will be returned to OSHC for their safety.
- I/We understand and agree that the information in this form will only be used in the above circumstances and will not be entered into the software unless needed.
- I/We understand that the normal After School Care charge will apply to this and this fee must be paid on the day via eftpos or no later than 7 days of the invoice being received.

Emergency Contacts and Authorised Nominees
In accordance with government regulations parents and guardians are required to nominate emergency contacts and authorised nominees to carry out the following responsibilities for their child.

Emergency Contact: A person notified in the case of an emergency where the parents cannot be contacted.

Authorised Collection Nominee: A person who has been given permission by the parent to drop off or collect the child from Westside Christian College OSHC. For identification purposes, a driver's licence may be required to be shown at pick up.

Authorised Medical Nominee: A person who is authorised for the administration of medications or to consent to medical treatment for the child.

Authorised Excursion Nominee: A person who is authorised to accompany the child outside the service premises.

- I / We understand that every effort will be made to contact me/us in the event of an emergency.

- We authorise the nominated supervisor or educator to provide any required first aid and to facilitate medical attention should this be required in the event of a sudden illness or emergency at the OSHC or whilst on an excursion off site. I/We give permission for OSHC staff to facilitate ambulance, medical or hospital services in the event of an emergency or accident involving my/our child. I /We accept and take responsibility for any costs associated with obtaining these services and/or any subsequent treatments.
Photographic and Video Permissions
Photographs and video footage may be used at times by authorised educators or support staff to document your child’s time at the OSHC. Please tick the following box to indicate your approval.

Enrolment Agreement
When enrolling my child at Westside Christian College OSHC/Vacation Care I understand and agree to the following:
1. I /We understand that all permanent Before and After School Accounts must stay in two weeks credit at all times.
2. All Fees are payable at all times including absences and family holidays unless the service is given a government directive to close.
3. All Fees are payable weekly and a service fee may be charged if payment is not made within 7 days of the invoice appearing on our software.
4. Casual Bookings for either OSHC or vacation care will be invoiced the week after the booking has occurred and will be payable within 7 days.
5. If I/We intend to terminate my/our child’s permanent enrolment at the OSHC or Vacation Care I /We understand and agree to giving at least two weeks written notice to the OSHC office or forfeit two weeks’ fees in lieu of notice. I/We are aware that if my/our child does not attend the OSHC or Vacation Care during the notice period CCS cannot be claimed and I/We will be required to pay full fees.
6. I/We understand that for a cancellation request for vacation care to be accepted, without a fee being charged, the cancellation must be made at least five working days prior to the vacation care program commencing. The cancellation fee is equivalent of the full fee if changes are made after this time.
7. I/We agree that if my child does not attend OSHC for a period of 13 weeks or more. Centrelink will cease my child’s CWA and I/We will be required to agree to another CWA before my child’s enrolment can be reactivated by Westside Christian College OSHC.
8. If my/our child is suffering an infectious illness or heavy cold that may be passed onto other children or staff I/We will not send her/him to OSHC or Vacation Care.
9. I/We agree to notify the co-ordinator promptly of any absences and give a reason for this absence. In all other cases, even due to sickness, I/We agree to pay the fees as a late cancellation fee.
10. I/We will ensure a responsible person drops my/our child to the OSHC or Vacation Care takes them to an educator prior to leaving.
11. I/We understand that I am responsible for signing my child/ren in and out of the service using the appropriate software.
12. If I/We are not able to collect my/our child from the OSHC or Vacation Care I will ensure that an authorised Nominee (as identified on page 9 of this enrolment form) will collect my/our child prior to the OSHC or Vacation Care closing.

  2. I/We understand that the OSHC or Vacation Care educators will not release my/our child to a person who appears to be under the influence of drugs or alcohol at the time of collector or who may pose any risk to my/our child.
13. I/We understand that if I/we are late collecting our child from the OSHC or Vacation Care the college reserves the right to charge a Late Collection fee. This will be at the OSHC Coordinators discretion.
14. I/We understand that my child/ren may be excluded from the program if their behaviour is unacceptable, a danger to themselves, others or the environment.
15. I/We agree to notify Westside Christian College OSHC immediately of any change in our or our emergency contacts, address’s and or telephone numbers.
16. I/We have read the OSHC handbook (available on the website or in the OSHC office) and agree to co-operate in all things to the best of my/our abilities. I have discussed the enrolment of my child with the Co-ordinator and understand the importance of family co-operation. I agree to be bound by the constitution/Rules and /or any bylaws of Westside Christian College OSHC.
17. I/We agree to abide by the Policies and Procedures of Westside Christian College OSHC.
18. I/We agree to the following privacy policy:
  1. Westside Christian College OSHC collects personal information, including sensitive information about children and parents or guardians before and during the course of a child’s enrolment. The primary purpose of collecting this information is to enable OSHC to provide a high level of care for your child. Some of the information we collect is to satisfy OSHC’s legal obligations, particularly to enable OSHC to discharge it’s duty of care.
  2. Certain laws governing or relating to the operation of OSHC require that certain information is collected. These include Public Health and Chid protection laws.
  3. Health information about children is sensitive information within the terms of the National Privacy Principles under the Information Privacy Act 2009. We ask that you provide medical information about children at the beginning of each year and when circumstances warrant.
  4. OSHC, from time to time, discloses personal information to others for administrative purposes. This includes government departments and medical practitioners.
  5. If we do not obtain the information referred to above, we may not be able to enrol or continue the enrolment of your child.
  6. Parents may seek access to personal information collected about them and their child by contacting OSHC.
  7. If you provide the College with the personal information of others, such as doctors or approved persons/contacts we would encourage you to inform them that you are disclosing that information to OSHC also. In doing this you give permission for both the College and OSHC to access the same information.
