Photographs and video footage may be used at times by authorised educators or support staff to document your child’s time at the Kindergarten. Please tick the following box to indicate your approval.

Sharing information

As this Kindergarten is located on the campus of Westside Christian College a request may be made by the school to obtain postal or email contact details. This is for sharing enrolment information or promotional information.

Permission to apply sunscreen and Insect repellent

When enrolling my child at Westside Christian College Kindergarten I understand and agree to the following:

1. I/We understand that in the sudden case of an illness or emergency the Director shall use discretion and take all reasonable steps to provide the appropriate medical attention for my child and I/we will be contacted as soon as possible. I/We understand that any costs relating to this treatment will be covered by us.
2. If my/our child is suffering an infectious illness or heavy cold that may be passed onto other children or staff I/We will not send her/him to kindergarten.
3. I/We will notify Westside Christian College Kindergarten when my child is absent and give a reason for the absence.
4. I/We will ensure a responsible person drops my/our child to the Kindergarten and takes them to an educator prior to leaving.
5. If I/We are not able to collect my/our child from the Kindergarten I will ensure that an authorised Nominee (as identified on page 7 of this enrolment form) will collect my/our child prior to the Kindergarten closing.
   1. I/We understand that the Kindergarten will not allow my child to leave the premises with a person who is not an authorised Nominee unless I have notified the Director in writing.
   2. I/We understand that the Director or responsible person in charge will not release my/our child to a person who appears to be under the influence of drugs or alcohol at the time of collector or who may pose any risk to my/our child.
6. I/We understand that if I/we are late collecting our child from the Kindergarten the college reserves the right to charge a Late Collection fee. This will be at the Director’s discretion.
7. I/We understand that Kindergarten fees must be paid in advance. Fees are payable at all times including absences and holidays. I/We understand that if my/our account does not have a zero balance each Monday prior to the statement run my/our account may incur an overdue account fee at the Director’s discretion.
8. I understand that if my child’s fees are not paid my/our child’s place at the Kindergarten cannot be guaranteed.
9. If I/We intend to terminate my/our child’s enrolment at the Kindergarten I /We understand and agree to giving two weeks written notice to the Director or forfeit two weeks’ fees in lieu of notice. I/We are aware that if my/our child does not attend the Kindergarten during the notice period CCS cannot be claimed and I/We will be required to pay full fees.
10. I/We agree to notify the Director in writing immediately of any changes in address details and/or phone numbers to either us as parents or to Emergency Contacts or Authorised Nominees.
